Adverse medication reactions (AMRs) continue to be a burden on our healthcare systems and more importantly on the quality of life for the patients under our care. Providing accurate documentation of our patients’ AMRs is an essential component of patient care. This program is intended to identify tools that pharmacists, pharmacy technicians, and nurses can utilize to identify potential AMRs. Additionally, AMRs are evaluated including those resulting from drug-drug interactions, demonstrating the value of medication reconciliation.
